病假条格式

1.居中写标题"病假条"，这是所有应用文的通用要求;

2.顶格写称谓，即向谁请假，注意应加上其职务，以示尊重;

3.第二行空两格开始写正文;

4.写明病假原由，这个地方要实事求是，否则后果自负;

5.病假起止时间，这个非常重要，必须写明确;

6.假期所去方向，联系方式(能第一时间联系上你的联系方式)，联系人;

7.祝福对方，这个是所有公文里表示对对方的友好;

8.最后加上请假习惯用语"请批准"、"请予批准"等;

9.病假人签名;

10.你写病假条的时间。
